25 September 2025, New York, USA / Geneva, Switzerland – As governments prepare to agree the Political Declaration of the Fourth UN High-Level Meeting on NCDs and Mental Health (HLM4), over three million people – united behind the Call to Lead on NCDs– are calling for stronger, faster, fairer action to prevent and control NCDs.

The NCD Alliance welcomes the Declaration as a renewed political commitment to address the world’s leading causes of death, disability and inequality. We recognise key steps forward, including the introduction of fast-track and tracer targets that signal the need for accelerated action, stronger recognition of mental health and neurological conditions, and more inclusive reference to disease such as oral and renal conditions.

Yet despite these gains, the Declaration falls short . It waters down language on proven, cost-effective prevention measures, omits essential commitments on health taxes, and weakens recognition of the role of civil society — limiting the very accountability that drives progress. In the face of rising needs and shrinking timelines, this lack of ambition is both disappointing and dangerous.

“Millions have spoken — and they are right to demand more. The time for half-measures has passed. We need committed leadership, clear accountability, and urgent investment to close the gap between commitment and change,” said Katie Dain, CEO of the NCD Alliance.

As the final five years of the SDGs begin, we urge governments to seize this moment:

  • Accelerate the implementation of proven NCD policies;
  • Break down silos and build more integrated systems;
  • Mobilise investment that matches the burden;
  • Deliver on targets with transparency and rigour; and
  • Engage the people most affected in decisions that shape their lives.

The Declaration is done. The world is watching to see who will truly lead.
